How To Receive Copies Of Your Medical Record

California law ensures patients access/disclosure of their medical information. The patient access law requires that a patient’s medical record be available upon a written request from the patient, or patient’s representative. Disclosure of a medical record must be accompanied by a patient consent/authorization. The records of your hospital stay are kept in the hospital Medical Records Department (also known as the Health Information Management Department).

During your stay at Centinela Hospital, you were issued a unique medical record number and all of your records were compiled into a unit record under that number. There is no charge if you have a need for a copy of your record for personal use. We will be happy to copy your record for any physician who is to provide continued medical care for your well-being at no charge.

Although we are staffed seven days a week for the processing of records, we are only open to the public from 8:00 a.m. to 5:00 p.m., Monday through Friday. We are closed evenings, weekends and holidays. The Health Information Management Department is located in the main hospital at 555 E. Hardy St., Inglewood, CA 90301. They can be reached by telephone at 310-680-8129.

To request your medical records from the hospital, please completely fill out the authorization form (English or Spanish) and provide a copy of your valid photo identification. You may send the documents to us by mail, via fax at 310-671-9487 or by email at CHMC-HIM@primehealthcare.com.

Please note: if your medical records contain sensitive information as listed in Section B of the authorization form, please check the appropriate box and initial next to the box.

How to Obtain a Copy of a Birth Certificate

The Los Angeles County Registrar-Recorder County Clerk’s Office provides copies of birth certificates for individuals born within Los Angeles County. Complete information on how to obtain copies can be found on their website:

Records are available for births occurring in Los Angeles County since 1866. Centinela Hospital does not provide copies of birth certificates. These must be requested in-person or via their website. Listed below are the locations to request copies of a birth certificate:
