Acute Rehabilitation Unit

At Centinela Hospital Medical Center we continuously focus on creating and providing opportunities that enable our patients to achieve their highest level of independence. With this goal in mind, the Centinela Acute Rehabilitation Unit has an interdisciplinary team of rehabilitation therapists, social workers and nursing experts who, under the guidance of a physician specializing in rehabilitation, assist patients in achieving their optimal level of function.

What to expect during your stay

  • A minimum of three hours of therapy, five days per week which may include:

    • Physical Therapy to help you regain strength, balance and coordination to restore mobility

    • Occupational Therapy to teach you new ways to perform activities of daily living such as bathing, dressing and grooming

    • Speech Therapy to assist you with communication and swallowing

  • Medical management of your disease and condition

  • Discharge planning by a social worker to prepare you for discharge

  • Family/caregiver education and training to prepare you to return home

What to Bring

It’s important to us that you feel comfortable and at home during your stay. We ask that patients and/or their families bring the following personal items for use in therapies and daily care:

  • Loose-fitting, comfortable clothing such as pants, shirts, socks, underwear, etc.
  • Sturdy, comfortable shoes with non-skid soles
  • A sweater or jacket with zippers or buttons (this is helpful for working on dressing goals)
  • Personal care items and cosmetics
  • Eyeglasses/contact lenses and supplies
  • Sleepwear and robe
  • Hearing aid(s) in a container labeled with the patient’s name
  • Dentures and a container labeled with the patient’s name
  • Assistive or adaptive equipment that you may have used at home. This will help staff members get a better idea of your prior level of function

We discourage you from bringing valuables to the Hospital.

Family and/or Support Person

During your stay on our Rehabilitation unit, your family and/or support person are welcome and encouraged to attend therapy sessions on a regular basis. There will be times when a family/support person will be required to attend therapy sessions, as determined by your Rehabilitation Team. Your Social Worker will let you and your family know when the need to attend therapies so that they can be adequately trained in how to help you when you leave the Hospital.

Discharge Planning

Discharge Planning begins the day you are admitted to the Rehabilitation Unit. Your physician and rehab team will meet with you and your family to plan appropriate discharge arrangements. Discharge from the Rehabilitation Unit does not necessarily mean that rehabilitation stops. Your continued recovery may include the services of home care nurses and aides, therapists or other outpatient services. Upon discharge from the Hospital, most patients will still initially require 24-hour assistance from a family member or support person.

A Typical Day on Our Acute Rehabilitation Unit

Morning – Nursing or therapy staff will assist with activities of daily living: bathing, dressing as needed. Nursing will assist with medications. Breakfast will be provided prior to the start of therapy sessions.

Noon – There will be a lunch break around noon. If necessary, a therapist will work with individuals that need help with eating/swallowing.

Afternoon – Therapy sessions will continue after lunch. Rest breaks will be provided in between sessions as needed. If any assistive devices are needed, therapy will help with the use and adjustment of the device/devices.

Evening – Dinner will be served and there will be time for visits with family and friends. There will also be time to read, relax and watch television.

Night – Sleep allows the body to recover from the day’s activities. Staff will check on patients throughout the night to provide any needed care. Every room is equipped with a call button If assistance is needed during the night.
